About Moving Forward Together

Managers, clinicians and planners are working together and are engaging with patients, service users and staff to develop plans for a better healthcare and social care system for people.

This new system of care will be organised in the most effective way to provide safe, effective and sustainable care to meet the current and future needs of the population.

It will be designed to support people to improve their own health, manage their own long term conditions and live independently at home for longer. People will be able to stay in their communities and access primary and community care services close to home. High quality hospital based care will be available when the required level of care or treatment cannot be provided in the community.

You will hear about progress to date and how staff and the public have been
involved in the programme. This is an opportunity to comment and ask
questions about how services move forward.
